Crayon Care/Financial Assistance Program

Since 1921, Scottish Rite for Children has treated patients regardless of their family’s ability to pay. We rely on the generosity of individuals, organizations, foundations and corporations to continue our mission.

We offer a financial assistance and charity care program called Crayon Care, which provides financial aid to qualifying families. 

The Crayon Care application form applies to all families regardless of income levels and insurance types (i.e. Aetna, Blue Cross Blue Shield, Cigna, United Healthcare, etc.). Please take the time to fill out the application so that we may better serve you. You may qualify for care at no charge or at a significantly discounted rate.

What is Crayon Care?

Crayon Care is a financial assistance and charity care program at Scottish Rite. It is not a form of health insurance, but it can help reduce out-of-pocket expenses for both insured and uninsured patients.

Who can apply for Crayon Care?

All Scottish Rite for Children families are welcome to apply for Crayon Care.

I don’t think I qualify for Crayon Care. Should I still apply?

It is not a requirement to apply for Crayon Care; however, we encourage all families to apply. Every family can qualify at some level, for either a discount on services with an annual cap OR a cap on total expenses incurred at Scottish Rite.

How much will I have to pay?

Families are limited to pay no more than 1% of their annual gross income for 12 months, but may qualify for 100% financial assistance, depending on their household’s annual gross income.

Do I have to renew my application?

Yes. Applications must be renewed on an annual basis.

Can I apply for financial assistance after my visit to Scottish Rite for Children?

Yes. Families can apply for Crayon Care at any time as long as there is an outstanding balance.

Parents are not in the same household. Which parent needs to complete the application?

The subscriber on the insurance needs to complete the application.

How do I apply for Crayon Care?

  • Complete the two-page application.
  • Provide proof of income for all household income earners.
  • Proof of 1% of your annual gross income in medical expenses within the last 12 months.
