How to Become a Patient
Thank you for considering Scottish Rite for Children for treatment.
Please note that international patients must first be formally accepted before traveling to receive care at Scottish Rite for Children.
To be considered for acceptance, patients must complete the International Services Request Form.
Once the request form is completed, you will receive an email confirmation that details next steps in our International Request process.
I’ve submitted the request form. What’s next?
- We will review your request to determine if treatment is within the scope of our services and if we can provide care to the patient.
- We require medical records as part of this review process. These medical records should be translated into English.
- These records include:
- Complete medical history
- All surgery notes
- All orthopedic notes
- All specialist notes
- Immunization and vaccination records
- Photograph(s) of patient showing area of concern
- These records include:
- The following records should be uploaded via our secure AMBRA Health platform.
- Imaging studies such as X-Ray, MRI, CT
- Imaging reports
- Our Family Services team will reach out to you to coordinate an initial screening telephone call to better understand the patient and his or her situation.
- We will decide about treatment of patient within approximately 60 days from this time.
- If the request is approved, tuberculosis (TB) test is required.
- Test results can be from skin or blood tests.
Additional Information
Please keep in mind that depending on your citizenship, you may also need to obtain a medical visa to be admitted into the United States for medical care. Learn more about the application process on the U.S. State Department website.
