Note: Online scheduling is available for new patients only. Use our Request Appointment form if you:

  • are an existing patient,
  • need to make an appointment with our Neurology, Pediatric Developmental Disabilities (PDD) or Rheumatology departments, or
  • are trying to be seen at our Frisco campus.
  • If patient has a permanent residence outside of the United States, follow request process for international patients. Click here.
