On this week’s Facebook live, Division Director Alex Cherkashin, M.D.* and Co-director of the Center for Excellence in Limb Lengthening David A. Podeszwa, M.D. joined us to discuss the devices used for limb lengthening and reconstruction. Below is a recap of the conversation.
History of the Ilizarov device
The original Ilizarov external fixator system has been around for about 60 years ago as a form of treatment to lengthen limbs and correct deformity. The external fixator has evolved to become more comfortable and easier for the patient to use/handle while wearing it.

The original Ilizarov external fixator system has been around for about 60 years ago as a form of treatment to lengthen limbs and correct deformity. The external fixator has evolved to become more comfortable and easier for the patient to use/handle while wearing it.
How has the device evolved?
- TRUE/LOKTM: first generation developed from Scottish Rite Hospital as a different iteration of the original Ilizarov equipment. It is more comfortable for the patient and easier for the surgeon to apply.
- TL-HEX: the most advanced external fixation system involving six struts to allow for more flexibility and precise adjustments for correction.
- Congenital conditions of limb length differences or deformities
- Post traumatic injuries
- The Ilizarov method was initially used for lengthening. However, through the years of innovation, the device has evolved to be a form of treatment for correction of limb deformity.
- If bones or joints need correction, the external fixator provides the best results.
- Cherkashin is a member of the hospital’s research team. Most of his work involves developing the software that builds these devices.
- The center’s research revolves around improving the devices, patient outcome studies and analyzing clinical results of patients who undergo this treatment.
- Members for our Center for Excellence in Limb Lengthening travel world-wide to train orthopedic surgeons on applying the external fixator.
- The equipment continues to become more complicated and proper education is necessary for best results in treatment for the patient.
